ALS — also called Lou Gehrig’s disease — attacks the motor neurons in the brain and spinal cord that send signals to muscles throughout the upper and lower body. Over time, patients lose voluntary control over movement. The condition progresses until swallowing, speech, walking, and eventually breathing all become impossible. Scientists still do not fully understand what triggers the disease, and no cure exists. It affects men slightly more often than women. The physicist Stephen Hawking lived with ALS for more than five decades, making him perhaps the most widely recognized face attached to the condition.

Little was publicly known about Randall’s illness during his lifetime. Bullock explained that her sister, Gesine Bullock-Prado, was the sole confidant “for a while.” Only later did the actress open up to close friends Jennifer Aniston and Amanda Anka, Bateman’s wife. The secrecy, compounded by the global pandemic of 2020, made the experience feel singularly lonely.

“I know why he asked me not (to),” Bullock said of the request for silence. “Especially during the global Covid-19 pandemic, bearing the weight of the diagnosis isolated me in the process.”

Grieving Before the End

Bullock and Randall, a former model turned photographer, made their relationship public in 2015. They co-parented Bullock’s son Louis and daughter Laila alongside Randall’s daughter, Skylar Staten Randall, from a prior relationship. Yet the actress revealed that Randall “was ill for half of our relationship,” meaning the couple spent roughly four years together while he was already fighting the disease.

“I started grieving Bryan four years before he passed,” she said. Randall died in 2023 at age 57.

The emotional toll, she explained, did not arrive all at once. While he was still alive, daily survival consumed every ounce of attention.

“My person left a lot earlier than the body left,” Bullock said. “I don’t think I ever dealt with that until after he passed, because you’re just on this treadmill.”

Caring Under Pressure

Looking back on the caregiving years, Bullock described a period of constant, quiet logistics — arranging medical support, managing household routines, and shielding her children from the full scope of what was happening. She noted that four nurses played a critical role in helping her look after Randall at home, and she expressed deep gratitude for what she called their “incredible” work.

Illness was not foreign to the Bullock household. Her father had suffered what she described as “horrible accidents,” and her mother battled cancer. That history, she said, gave her a kind of hardened composure.

“It doesn’t scare me — not the illness itself, it doesn’t scare me. I can see and be around just about anything,” she said. “But I had two young kids that were navigating it, especially a little girl who saw (Randall) as a father figure.”

That last detail — a young child watching a father figure diminish — was, in her telling, the hardest part of the entire ordeal. She praised her children for how they carried themselves through the period and acknowledged the emotional labor of keeping the household steady while the disease advanced.

Returning to Work

Earlier this year, Bullock told People magazine that after Randall’s death she turned her energy toward healing, comforting her children, and making sure her family was “in the best possible place” before she re-entered the professional world. That re-entry has been visible this summer as she has promoted Practical Magic 2, the long-awaited sequel to the 1998 romantic-fantasy film. Bullock reprises her role as Sally Owens in the story of a multi-generational witch family, and the movie is scheduled for theatrical release next month.
